This form should be used for customers age 12 and younger. Request for service does not guarantee field trip availability. The field trip coordinator will confirm availability within one week of receipt of this request form. Requests should be made 30 days in advance.Keep in mind that the programs are in a classroom style setting. Program providers prefer a class size of no more that 40-50. If you have a larger group attending please note that we may have to split it up into different sessions on the same day or on different days.
